The report summarizes the results of the Special Project on Facility Closures/Phasedowns. A history of mental retardation institutions in America is provided, and 24 closures of State-operated mental retardation institutions are described. Studies of the impact of closure and involuntary relocation on patients, their families, and closing facility employees are reviewed. In sum, facility closures and client relocations are found to frequently result in physical-behavioral stress reactions; however, these effects seem to be stronger for some groups and occur only under certain circumstances. Families faced with the relocation of their relatives out of large state institutions have reported a high degree of stress and have strongly resisted these transfers. Nonetheless, findings indicate that family attitudes can be changed and that proper attention to family members' perceptions and needs could alleviate the stresses they experience. Institutional closures also affect the morale and performance of staff; however, suitable administrative policies and clinical guidelines can be adopted to cope with these negative effects. The Dixon Closure Study is described and guidelines are presented for encouraging positive outcomes for clients, families, and employees involved in closures.

This report examines two measures that identify children with emotional and behavioral problems: high scores based on questions in the brief version of the Strengths and Difficulties Questionnaire (SDQ) and a single question about serious (definite or severe) overall emotional and behavioral difficulties. Children were classified into four groups, those with: only high scores on the brief SDQ, only serious overall difficulties, both high scores on the brief SDQ and serious overall difficulties, and neither high scores on brief SDQ nor serious overall difficulties. Childrens characteristics, conditions, and service use in these four groups were compared.

The historical background to the brain operation involving destruction of the anterior portion of the frontal lobe, called prefrontal leukotomy, is briefly mentioned. A general evaluation of the benefits and defects of the operation is presented, followed by a list of disease categories for which the procedure might be considered. Such symptom complexes or disease categories as manic-depressive psychosis and involutional melancholia, obsessive-compulsive neurosis, schizophrenic psychosis, paranoid psychoses, psychosis associated with organic disease of the brain, epilepsy, psychopathic personality, intractable pain and physical defect states and suicidal tendencies are all considered, and the degree to which they might benefit from the operation is described. The operative approach and technique is briefly mentioned. The postoperative course and the importance of post-operative care to reach a satisfactory outcome is discussed. A suggested reading list of 13 items is included.

A second message of the report is that mental disorders are real health conditions that have an immense impact on individuals and families throughout this Nation and the world. The body of this report is a summary of an extensive review of the scientific literature and of consultations with mental health care providers and consumers. Contributors guided by the Office of the Surgeon General examined more than 3,000 research articles and other materials, including first-person accounts from individuals who have experienced mental disorders.

The joint sponsorship of the workshop by the National Institute of Child Health and Human Development (NICHD) and the National Institute of Mental Health (NIMH) indicates very clearly the importance that both Institutes attach to the issue of behavior problems in people with mental retardation. Both Institutes have tried to respond to growing recognition that, for many individuals, it is not their mental retardation that keeps them from being placed satisfactorily in the community. Rather, it is the behavioral problems associated with that mental retardation.
